The two most common filings can be completed online below. Everything else is in the document library at the bottom of this page, or at the counter at 44 Compost Court, where the pens are attached to the desk for reasons documented in the FY 2019 annual report.

Claim Your Tree (Form LR-1)

Registration ties your tree's parcel number to your household so your leaves come back to you and not to an address that merely resembles yours. Registration is required once per tree, not per year. Trees registered before 2004 were carried forward, mostly.

Misattachment Complaint (Form LR-7)

If your tree has been dressed with the wrong leaves, file below. The most common finding is oak on elm, which the Department acknowledges and regrets. Photo evidence moves your complaint up the queue. Complaints about leaf orientation, leaf spacing, or a leaf that "looks tired" are heard by the Board in the order received, which is to say eventually.

Forms not yet approved for online filing may be printed and delivered to the counter. Double-sided printing is permitted as of the March vote.
